The hospital sent my bill to collections. Is it too late to ask for help?

No. Charity care can usually still be applied for after a bill leaves the hospital, and medical debt in collections has protections ordinary debt does not.

No, and this is the point at which most people stop trying — which is what makes it worth saying plainly.

A bill moving to a collection agency changes who is calling you. It does not, on its own, end your eligibility for the hospital's financial assistance.

Charity care does not expire when the bill moves

Every non-profit hospital in the United States is required to have a financial assistance policy. Those policies generally allow an application for a period after the bill was issued, and an account already in collections is frequently still within it.

The application also has to go back to the hospital, not the collection agency. The agency did not grant the discount and cannot; it bought or was assigned the account. Call the hospital's billing or financial counselling department directly and ask for the financial assistance application by name.

If assistance is granted after the account has been sent out, the hospital is generally expected to pull it back and to correct what was reported.

Medical debt is not ordinary debt

Two things are true of medical collections that are not true of a credit card:

  • It carries no interest while the hospital holds it, and moving it onto a credit card to make the calls stop converts it into ordinary high-interest debt permanently.
  • It has limited effect on credit scores, and paid medical collections and smaller balances are treated differently from other debt.

Demand validation, in writing

You can require a collection agency to validate the debt in writing — what it is for, how much, and who they are collecting for. Do this by post or email rather than on the phone, and keep proof of sending.

It is worth doing even when you are sure the debt is real. Accounts get sold with errors in them, the amount is often not what the hospital's own itemised bill says, and an account you are formally disputing should not be reported while the dispute is open.

And before you agree to a payment plan

Ask about Emergency Medicaid. Every state runs it, it pays for emergency treatment for people who meet the income rules but not the immigration ones, and it can be applied for after the treatment. It is one of the few things that can clear an emergency room bill entirely, and a payment plan you have started does not stop you asking.
